The Connecticut State Medical Society-IPA, Inc. (CSMS-IPA) today announced that it has joined with the American College of Physicians (ACP) to make important online software available to primary care practices interested in pursuing the Patient Centered Medical Home (PCMH) model of care. The ACP developed Medical Home Builder 2.0 to help physician practices improve patient care, organization and workflow. CSMS–IPA is making the MHB 2.0 Tool available at no cost to eligible primary care practices participating in the IPA’s Patient Centered Practice Model (PCPM) program to help them improve their ability to provide more efficient, coordinated care.
